CHOOSE

 Choose a health & fitness activity for your specific group or organization. We’ve provided three ways to help you, if needed.  First, look for some suggestions on our Activities page; second, come to one of our bi-weekly meetings to hear what others are doing; or third, contact us with questions.

 DO

 Do your activity as a group during the suggested campaign timeframe (April 1 – April 30, 2008).  If you like, you can stop by one of our bi-weekly support and planning meetings to share how things are going or contact us if you just need a motivational booster.

If you’re already an active group or have activities in place before, during or after the Campaign “kick-off” period – Well Done and Keep Going!

 TELL

 Tell your story!  We want to recognize your efforts.  We want to count what you do, because what you do counts!  We’ve created three ‘fill-in-the-blank’ forms for you to tell us about your group’s efforts and accomplishments.

  1.  The Get Up & Go Participant form – this is just to tell us a little about yourself and to identify what activity your group wants to do.
     

  2. The Get Up & Go Activity Completion form – this allows you to give a final update about your activity.  Did you do what you set out to do?  How many members of your group or organization participated?  We want to use the information from this form to award recognition certificates and get a final update of the campaign’s overall success for a celebration event that we are planning in May.
     

  3. The Get Up & Go Highlight Reel – this gives groups and organizations an opportunity to really wow us with their success stories!  This form is a little more open ended and gives you the option to “toot your own horn a little bit” – but only in such a way that you inspire others to do the same.  Examples of success stories may be related to attaining a group goal to lose weight (like a “Biggest Losers” contest) or establishing and practicing a new company policy for health & fitness, or starting a neighborhood trail and walking program. 

    Tell us what challenges you were able to set and meet for the sake of health & fitness, and we’ll share your story with others.
